1. General information
The Ungava Tulattavik Health Centre (CSTU) is responsible for the personal information collected through this website. This page explains what is collected on dentistnunavik.com, why, who can see it, how long it is kept, and how you can exercise your rights.
This site exists for one reason. The Kuujjuaq dental clinic is closed for major renovations, and the triage form allows you to describe a dental problem so that the urgency can be assessed and you can be contacted about care.
This policy covers dentistnunavik.com only. The CSTU’s general privacy policy applies to its other services and is available at tulattavik.com.

2. Definitions
- Personal information means any information that concerns a natural person and allows that person to be identified, directly or indirectly.
- Health information means personal information about your state of health, your symptoms, your allergies or the medication you take. Under Quebec law it is sensitive information and is given heightened protection.
- Service provider means a third party that processes information on behalf of the CSTU, under contract and on its instructions.
3. What is collected
3.1 The triage form
When you submit the triage form, the following is collected:
- Identification: first name, last name, gender, date of birth.
- Contact details: address, community, province, postal code, telephone number, email address.
- Dental and health information: whether you are in pain and how severe it is, the tooth or area of the mouth involved, problems with your gums or tongue, when the problem began, allergies, pregnancy, and any medication you are currently taking.
Your health insurance number is not requested and should not be entered in the form. Your IP address is not stored with your submission.
3.2 Technical data
The web server keeps standard access logs (IP address, date and time, browser, pages requested) for security and troubleshooting. These logs are kept for no more than 60 days and are not linked to form submissions.
3.3 What is not done
This site uses no analytics service, no advertising, no social media tracking and no profiling technology. The information collected is not used to draw inferences about you, is not used for marketing, and is never sold, rented or traded.
4. Why it is collected
- To assess how urgent your dental condition is.
- To contact you in order to arrange care, or to refer you to the right service.
- To keep a temporary record of requests so that none is lost while the clinic is closed.
- To protect the form against automated abuse.
5. Who has access
Access is limited to the dentist and dental team of the Kuujjuaq clinic and to the CSTU staff assigned to dental referrals. Notifications are sent to a CSTU address hosted on Government of Québec infrastructure.
Each person has an individual account, protected by two-factor authentication and limited to viewing submissions. Account activity is logged.
5.1 Service providers
- Alexem Studio inc. (Québec) — design, hosting and technical maintenance of this site. The data is stored on a server located in Canada.
- Cloudflare, Inc. (United States) — the security check that confirms the form is being filled in by a person rather than a machine. It receives your IP address and technical signals from your browser at the moment you submit. It does not receive the content of the form. Because this provider is located outside Québec, the transfer is limited to what the security check requires.
5.2 Legal disclosure
The CSTU may disclose personal information where the law requires or permits it, in particular to comply with a court order or a legal obligation, or to protect the safety of a person.
6. How long it is kept
Submissions are permanently deleted from this website 60 days after they are received.
If your request leads to an appointment or a referral, the relevant information is transferred to your file at the CSTU. From that point it is governed by the CSTU’s retention rules and by the legislation applicable to health and social services information, not by this policy.
7. Consent
The form cannot be sent until you give your express consent. That consent is specific to the purposes described in section 4 and is requested separately from the rest of the form, because health information is sensitive.
You may withdraw your consent at any time by writing to the person in charge named in section 1. Withdrawing consent does not undo care that has already been provided.
If you complete the form on behalf of another person, you must have obtained that person’s consent. For a child under 14, the form must be completed by a parent or guardian.
8. Your rights
Under Québec law you have the right to:
- be informed of the personal information held about you and obtain a copy of it;
- have information that is inaccurate, incomplete or ambiguous corrected;
- withdraw your consent;
- request that information about you stop being disseminated, or that a link to it be de-indexed;
- receive the information you provided in a structured, commonly used technological format;
- file a complaint with the Commission d’accès à l’information du Québec.
To exercise these rights, write to the person in charge named in section 1. A response is provided within 30 days.
9. Cookies and tracking technologies
This site uses only what is strictly necessary for it to work:
- a language preference stored when you choose English, French or Inuktitut;
- a short-lived technical token used by the security check while the form is being submitted;
- a session identifier for CSTU staff signed in to the administration area.
There are no analytics, advertising or profiling technologies on this site. Nothing here builds a profile of you or follows you to other sites. You can refuse or delete these items in your browser settings; the only effect is that your language choice will not be remembered.
10. Security measures
The following measures protect the information collected here:
- the entire site, including the form, is served over an encrypted connection (HTTPS);
- two-factor authentication is required on every account that can view submissions;
- accounts are restricted to viewing submissions and cannot modify the site;
- account activity is logged;
- submissions are deleted automatically after 60 days;
- IP addresses are not stored with submissions;
- outgoing email is authenticated (SPF, DKIM and DMARC) to limit spoofing;
- the site is protected against automated attacks and abusive traffic.
No system can be guaranteed absolutely secure. If you believe that an incident has affected your information, contact the person in charge named in section 1.
11. Changes to this policy
This policy may be updated to reflect changes to the site or to the applicable legislation. The date at the top of the page indicates the last update. Significant changes will be indicated on the home page.
12. Links to other sites
This site links to tulattavik.com and may link to other organisations. Those sites have their own privacy policies, for which the CSTU is not responsible.
13. Contact
Any question about this policy, about the information held about you, or about a possible incident should be sent to the person in charge of the protection of personal information named in section 1.
For a dental emergency, do not use email. Go to the Kuujjuaq hospital or call 819-964-9090.
